Informed about a clinical incident regarding failure of communication between non-resident on-call registrars and ward team out of hours.  Conflicting statements with no evidence to support or refute as clinical notes deemed invalid due to use of correction fluid and potential for being falsified. ‘Medical Defence Union’ had advised a significant governance issue with the Trust leaving junior doctors vulnerable.  Add-on system to handover database with a secure call-log facility for out of hours record of telephone summaries while non-resident.

Part of a Trust-wide initiative.  Following success of spinal database, approached to create an electronic handover for junior ward doctors, as well as a system for handover of patients at SHO and middle-grade levels overnight and at weekends.  Database back end and front end completed.  Training and roll out completed.  option for inclusion of an electronic handover referral add-on for Critical Care Outreach and Orthogeriatric Service in the future (code written, awaiting testing)

Quality of Healthcare Records and Consent Process Initiative – Reporting and Educating

Part of a Trust-wide initiative.  Involved in central committee planning, along with creation of a spread-sheet tool to allow data validation and automatic analysis and report generation.  First round of data collection taken place and presented at local department Directorate Meeting.  I have subsequently brought together outside agencies to improve the overall efficiency of this project.  Expanded to include multiple domains and combine data from all 5 divisions into a central pre-programmed spreadsheet – represented in 2015 & 2016

Spinal On-Call Referral Database

I quickly recognised an inefficient system or recording and managing on-call referrals in the busy tertiary Spinal Unit, and developed a database with a user-friendly front-end for recording referrals and allowing easy searching.  This led to development of a printed discharge summary for patients transferred from the community for assessment, to allow better communication with primary care clinicians. Presented at International Forum of Quality and Safety in Healthcare (London 2013).

Trauma Handover and Operation Note Computer System

With the move to shift based systems in keeping with EWTD directives and the transfer of emergency surgery from City to Sandwell Hospitals, more clinical errors were noticed within the department relating to poor patient handover on admission and peri-operatively.   A trauma database was designed with input from every level of staff within the Orthopaedic department.  The system incorporates patient admission details with typed operation notes, and has the ability to capture adverse events for presentation at monthly mortality and morbidity meetings.  The database was successfully implemented in the Trust’s department.

Teaching

 

I hold a Post-Graduate Certificate in Medical Education from the University of Dundee, and employ different models of teaching ranging from didactic lectures, facilitated learning, group discussion, bedside examination and simulated training.  I enjoy teaching and feel it is a vital part of medical practice to hand down skills to juniors and allied health professionals, as good clinical care depends on the coordinated efforts of different medical disciplines.
